Pharmacy Cost of Dispensing Survey
Pharmacies are required to participate in this survey.
Survey packets were mailed to pharmacies located in the state of West Virginia that participate in the West Virginia Medicaid pharmacy program on July 7, 2026. Below are the notification letters and the Excel version of the cost survey tool. Cost of dispensing surveys are due by August 18, 2026.
