Myers & Stauffer serves as a contractor for the Oregon Health Authority, the agency that administers the Medicaid Program for the state of Oregon. This webpage serves as a centralized resource for a means to obtain information related to the average actual acquisition cost (AAAC) program.
Pharmacy Prescription Volume Survey
All pharmacy providers are required to participate in the survey pursuant to OAR 410-121-1055 and 410-121-0160. Reported information will be used to determine your dispensing fee tier assignment for the following calendar year. Each year, surveys will be distributed starting August 1st and will be due by August 31st. Providers who fail to participate in the survey will be placed in the lowest dispensing fee tier.
Providers should fill out the form for all pharmacy NPIs with their organization that are enrolled with Oregon Health Authority. For providers with 5 or more pharmacy NPIs, please complete the multiple store submission template and upload the ‘multiple submissions’ option at the survey link.
